 If your team is already scheduled to train with us you can fill out the individual registration and medical release here.

This form will need to be filled out by individuals coming to a Flex Training Session for the first time and do not have a Flex Pass Package or by players who are coming in with their teams.

Please print out form, fill out the appropriate information, and bring it with you when you come to Total Hockey.
